The Inner Sanctum instance, which was added with patch 14.9, is a high level instance geared towards players of level 125+ (though, rest assured that level 125 players won't last very long in there). As the "extension" of the Temple of Three Winds, this instance is similar in theme to the TotW, housing a mysterious cult of Hezak. Great adventures and treasures can be found inside the Inner Sanctum.
Location
The Inner Sanctum can be reached by handing an Exarch Robe from the TotW to the Blind Cultist, standing next to the temple. In return she will hand you a Sealed Inner Sanctum Pass. Right-clicked, this returns an Inner Sanctum Pass which can in turn be used on the portal next to the Blind Cultist, to be teleported to the Inner Sanctum. Inside the Inner Sanctum, you can get Inner Sanctum Knowledge parts. The top and bottom half of these drop from two side bosses found in the second room, and combined result in the permanent key Inner Sanctum Knowledge, which will not disappear when used on the portal, contrary to the Inner Sanctum Passes, which disappear once used. All the robes from inside the Inner Sanctum may be exchanged in the same manner as the Exarch Robe.
Rites of the High Exarch can be purchased from Holo Deck in ICC HQ for 10 ICC Commendations (earnable from the same Holo Deck, from quests handed out by Arbiter Vincenso Palmiero and Arbitration Drone present there every other week) and equipped in Hud1 slot to prevent all non-boss mobs from aggroing a player on sight. Bosses outlined below however will still attack all players on sight regardless if this item is equipped. Non-boss mobs will only aggro a player with the item equipped when that player attacks any mob in the vicinity. This can be useful in pulling a boss mob away from their non-boss adds, since the adds do not automatically aggro a player just because their boss aggroed that player.
To move between the different rooms in Inner Sanctum, players need to either step into or use one of the portals. The portals will stop working for any player that is aggroed by any mob. Most portals are static and can be used by any player, except the portal from first room into the basement that temporarily spawns for 2 minutes at the remains of Lingering Soul and can be used by players with levels 125-200, and the portal from the first into the second room that temporarily spawns for 2 minutes at the remains of Fezael the Watcher, usable by players with level 125+. A static portal that teleports players from the second to the third room requires Inner Sanctum Knowledge to be used to activate it.

Tactics: After all of the mobs surrounding Iskop has been cleared, position your team directly behind the ramp near Elzak the Confessor. Your pulling team should consist of typically 2 people: a tanker and a disabler capable of snaring or rooting. An ideal disabler would be a fixer capable of AOE snares. Allow the tank to aggro Iskop first. This will immediately spawn 3 Pandemonium Idols around him. The disabler should then proceed to snare or root all 3 idols before the tank pulls Iskop towards the rest of the team waiting behind the ramp. This effectively narrows the fight to Iskop himself.
Iskop hits moderately hard, but not beyond 1000 damage per hit. He does however have the ability to periodically summon 2-3 fanatics in the middle of the fight. These fanatics do not deal much damage and only have approximately 8000 health, but will explode after a short amount of time to deal significant damage to the entire team. This self-destruct nano can be identified when the fanatic begins to have glowing red lines lashing out of him. As such, when Iskop spawns the fanatics, all DPSers should immediately swap their damaging priority on these fanatics. It is usually a good idea to call out to the team whenever the fanatics are spawned, so the team is informed to switch targets.
As long as the disabler remembers to refresh any short running roots or snares on the Idols, and the team is able to burn down the fanatics before they explode, the fight should not be much of a challenge to a properly equipped team.

Tactics: Before beginning the fight, throw any short term buffs on your entire team that may aid in the battle. Temporary health buffs and short term HoTs are useful in sustaining the team in this substantially challenging fight. When you are ready, get the tank to pull Ummoh quickly to the portal, where your team should be waiting.
Once pulled, Ummoh will apply a minor DoT/Debuff on the tank that will eat away at his health for 200 every 2 seconds, in addition to lowering his Offensive/Defensive Modifiers. Shortly into the fight, Ummoh will cast his signature nuke, Immortal's Havok, on the entire team. This nasty nuke will deal 1100-2400 damage 3 times over 6 seconds. Ummoh will however chain this nuke consistently throughout the entire fight, and this can be extremely deadly to the lower healthed team members. It is estimated that Ummoh casts Immortal's Havok once every 20 seconds. In addition, Ummoh will cast a damage buff on himself every 3 times he casts Immortal's Havok, allowing him to dish out additional damage on the tank. He will also refresh the minor DoT/Debuff on the tank.
The fight is indeed extremely healing-intensive, and success very much depends on the healing capabilities of your team. A skilled doctor is essential for keeping the health bars of each team member topped, and a well timed Alpha & Omega may save the entire team from certain death. However, his main concern should be keeping the tank sustained. Off-healers such as adventurers, MAs or MPs should also make an effort in topping off the health of the team before Ummoh recasts Immortal's Havok.
Since Ummoh's nuke cycle is highly predictable, it may be wise to use any manner of protection just before Ummoh casts Immortal's Havok. A NT's Nulity Sphere (MK II) is capable of absorbing one in three nuke cycles, while a soldier may sustain TMS to stave off most of the damage. If the team is incapable of restoring each member's health every 20 seconds, absorption shields should be thrown on team members to soften up the incoming damage.
Lastly, like all other Inner Sanctum bosses, Ummoh also has the ability to periodically spawn 2-3 fanatics to slap another layer of difficulty to the already challenging fight. Since the fight tends to be exceedingly chaotic, it is imperative that team members call out the fanatics when they spawn so DPSers can switch targets. Focus DPS on fanatics that are glowing red first, and do not allow any fanatic to detonate or the fight will almost certainly end.
The fight should not be attempted with characters below 5000 health.

Singling out Inobak is essential and should any extra add be pulled, immediately take flight to a pillar to cut off your line of sight (LOS). This will instantly cause your pursuers to lose aggro. It should be noted however, that Inobak tends to root his target with a nuke, so in the case that the tank fails to pull Inobak, he will likely be faced with instant death. Since Inobak may take a while to kill, you may want to pull him to the entrance, or if you are confident of your team's capabilities, you can burn him down at any side of the room.
The majority of the fight is a simple tank and spank. Inobak usually chains a single target nuke on the tank that will almost certainly root him for the entire fight. Each nuke does 1300-2200 damage, and roots for 11 seconds. If the team desires to pull Inobak further away, it is suggested that an off-tank overtakes Inobak's aggro and run as far as possible before Inobak lands the root-nuke on him instead. In addition, Inobak does periodically cast his signature AOE nuke on the entire team every 40 seconds or so (indicated by the fixer-grid animation). It hits for about 1000-2000 points of cold damage, and the doctor could have problems keeping the entire team healed while sustaining the tank at the same time. It is essential, therefore, for any secondary healers to negate this AOE damage, considering that the damage is typically minor and easily out-healed by adventurers and MAs.
The fight should not be attempted with characters below 3000 health.
